Intensive Physical Therapy Application

This intensive therapy application is to be filled out for those interested in participating in an intensive physical therapy program. These programs are 5 days per week for 2-4 hours per day for 2-4 weeks. After the application is completed. The director of physical therapy will review your application to determine the proper program for your child and you should expect a phone call from the staff at the front desk.
