Intensive Physical Therapy Application

This Intensive Therapy application will be filled out for those interested in participating in an Intensive Physical Therapy program. These programs are five days per week for 2-4 hours per day for 2-4 weeks. After the application is completed, the director of physical therapy will review your application to determine the proper program for your child, and you should expect a phone call from the staff at the front desk.
